The Honolulu C.C. Office for Students with Disabilities is conducting a self-study and program evaluation this term. As a student who receives our disability services, we seek your perceptions, evaluations, opinions, suggestions, and recommendations.

We realize that this is a rather long survey, but we know of no other efficient way to obtain the feedback of the students to whom we provide services.
