HCC Questionnaire on Leadership and Governance SP '05

This questionnaire is going to be used by the Accreditation Standard IV (Governance and Leadership) team to measure the thoughts and opinions of the Faculty and Staff at Honolulu Community College. The team plans on keeping track of who has responded but will not keep track of how any individual member of the faculty or staff has answered any questions.

We plan on reminding those who have not responded until they do. That's why we put your name on the bottom of the sheet.
