Each year, the Japanese Cultural Center of Hawaii (JCCH) Board of Directors evaluates the performance of its Executive Director/President.
We have the opportunity to solicit feedback from multiple JCCH constituencies through this online survey.
What follows is a job description for Lenny Andrew, the President and Executive Director of JCCH. The survey asks you to rate his performance in each category using a five-point scale.
Please choose the appropriate number rating based on your experiences and observations of the President/Executive Director during the past 12 months. To do this, simply click on the appropriate number on the number bar line for each job task.
If you feel you are not able to accurately make an assessment of the President/Executive Directors performance in a given category or job task, choose NA (Not Applicable).
We encourage you to include any comments or recommendations in the comments/recommendations section of each category.
In an effort to solicit candid feedback, your responses will not be personally identifiable, but instead will be aggregated with all other survey results.
Personally identifiable information in the comments section will be omitted.
However, a login procedure is needed to ensure that you are a valid respondent and to prevent multiple unsolicited assessments. You may terminate filling out this survey at any time and log back in at a later date to complete the survey.
Once you click on the PAU button at the end of this survey, you will see a THANK YOU message on the screen. At this point, your responses will be anonymously stored and you will be unable to return to the survey to change or delete your responses because it will be impossible to identify your responses from all other responses.
Thank you for your participation in this assessment and for your candid feedback. Survey results will enable the President/Executive Director and the Board of Directors to set our course for the future.
Mahalo for your kokua,
Eric Martinson
Chairman of the Board
